Lie

relationship long axis of fetus to long axis of mother


  1. Longitudinal lie (99.5%) – Long axis of fetus is parallel to long axis of mother (commonest type).

  2. Transverse lie long axis fetus is crosswise axis of mother. 

  3. Oblique lie - long axis of fetus is oblique axis of mother.

fetus Attitude

  • Relationship of fetal head to spine.

  • Flexion (Universal / normal attitude). 

  • Abnormal - Extension and neutral (military).


fetus Presentation 

  • part of fetus which occupies the lower pole of uterus (pelvis brim). 

  • part of fetus felt to examining finger during vaginal or rectal examination.

  • TYPE

1. Cephalic (96.5%) - Commonest type 


2. Breech or podalic (3%) 


3. Shoulder (0.5%) 


4. Compound presentation( head with hand)
- if presenting more than one part of fetus.
Commonest etiology- prematurity


  • Presenting part 

  • part of presentation which overlies the internal Os (felt by examiners finger). 

  • Presenting part decide by the attitude of fetus.

fetus Position 


Relationship between a specified bony landmark (maternal pelvis ) or fetal presenting part. (pelvis is divided in 8 equal segment of 45° angle).



 Type of position

Different position with-vertex presentation and longitudinal-lie.

 


(i) Left occipito-anterior (LOA) - occiput present anteriorly and towards left. 

This is the most common position and lie.

 

(ii) Right occipito-anterior (ROA)- occiput present anteriorly and towards right.

Less common than LOA, but not associated with labor complications. 


(iii) Left occipito-posterior (LOP) - occiput present posteriorly and towards left. 


(iv) Right occipito-posterior (ROP) - occiput present posteriorly and towards right. 


(v) Occipito-anterior (OA) -  occiput present anteriorly (absolutely straight )


(vi) Occipito posterior (OP) – occiput present posteriorly (absolutely straight). 


(vii) Left occipito- transverse (LOT) - occiput present transversely and towards left. 


(viii) Right occipito- transverse (ROT) - occiput present transversely and towards right.


  • Different position with breech presentation and longitudinal lie: 



  1. Left sacrum anterior (LSA)- sacrum , which present anteriorly and towards the left. 

  2. Right sacrum anterior (RSA)- sacrum present anteriorly and towards the right. 

  3. Left sacrum posterior (LSP)—sacrum present posteriorly and towards the left. 

  4. Right sacrum posterior (RSP)—sacrum present posteriorly and towards right. 

  5. Sacrum anterior (SA)—sacrum present anteriorly. 

  6. Sacrum posterior (SP)--sacrum present posteriorly.
